Renal Mechanisms of Diuretic Resistance in Congestive Heart Failure

چکیده
This study reviews the renal aspects of diuretic resistance occurring in treatment, mostly with loop diuretics congestive heart failure. A short discussion on different classes diuretics, including recently introduced sodium-glucose transporter 2 inhibitors, and their mechanism action nephron is provided, followed by a summary recent data discussing causes pathophysiological mechanisms resistance. The major cause appears to be localized within distal tubule. Traditionally, concept compensatory post-diuretic sodium reabsorption (CPDSR) was considered resistance; however, studies have disputed this traditional demonstrated that patients failure are constant sodium-avid state. Finally, options therapeutic strategies, combining summarized.

Mechanisms and management of diuretic resistance in congestive heart failure.
Diuretic drugs are used almost universally in patients with congestive heart failure, most frequently the potent loop diuretics. Despite their unproven effect on survival, their indisputable efficacy in relieving congestive symptoms makes them first line therapy for most patients. In the treatment of more advanced stages of heart failure diuretics may fail to control salt and water retention de...

During a period of four years between June, IS, 1988 to June, IS, 1992 one hundred and fourteen patients with congestive heart failure (CHF) were admitted to the pediatric department of Taleghani General Hospital. During the above period, 192 patients with heart disease were hospitalized at this department and CHF was the cause of admission in 59.4% of them. Congenital heart disease (CHO) w...

The fundamental role of sodium in maintenance of fluid homeostasis and progression of heart failure (HF) has been a central paradigm in medicine for more than a century. As a result, a core treatment principal in HF management has been the avoidance of dietary sodium. However, accumulating evidence ranging from the basic sciences to epidemiological studies has challenged this paradigm, suggesti...
متن کاملDiuretic-resistant congestive heart failure treated successfully with peritoneal ultrafiltration.
We present a case of successful peritoneal ultrafiltration (pUF) treatment in a 60 year-old patient diagnosed with diuretic-resistant congestive heart failure fulfilling the criteria for type 2 cardio-renal syndrome. Six months of pUF treatment with one daily dialysis exchange with icodextrin as an osmotic agent resulted in better functional status (from IV to II/III NYHA class), quality of lif...
متن کاملAbnormal Baroreflex Mechanisms in Congestive Heart Failure
P atients with low output congestive heart failure have less-than-normal slowing of heart rate during pharmacologic increases of arterial pressure, indicating impairment of the arterial baroreceptor-heart rate reflex.1 The mechanism underlying this abnormality is not fully understood and could involve one or more components of the reflex arc, including an afferent limb (generation and transmiss...
